SpamMotel
SpamMotel creates unique email addresses for you to use when providing your email address. Reminders are then attached to each message sent to that address. You can then choose to receive or not receive email sent to each address. The service can be used entirely on the Web, but they have a downloadable interface which makes it much easier to use.
