If you get a page saying "one offer per household" it means you have requested a sample before using your address.

The free sample offer is available in the US to those who have not requested an Arm & Hammer toothpaste sample before.
What are the risks in requesting the free sample?:
Church & Dwight has a good privacy policy, but you are free to use a junk email address and you don't need to enter your phone number. I was a little concerned since the offer is no on the main company site, but I did confirm that the offer site is owned by Church & Dwight, makers of Arm & Hammer products.
