Visit the Boss Skincare site and click on the "sample" link in the top menu. Fill out the form with your name, address, gender, date of birth, and email address. Please note that you need to enter the date in DDMMYYYY format instead of the month first format that we in US are accustomed to. See the notes below about not using your real birthdate.
